Fill out our Registration Form.

Enter your contact information and fund preferences through our secure registration form.  You will be asked to electronically sign a waiver before you can create your fund.

Be the First to Donate.

As the creator of your special fund, you need to be the first person to donate. Your donation sets the example for others who will also be contributing to your fund!  We request a minimum donation of $10.00.

But, how much you give, is less important than the fact that you gave. When asking others to give, it is always important that you be able to say to them, "This cause is important to me, and I personally support them."

Confirm your Registration.

Once you make the first donation to your fund, a confirmation message will be sent to the email you specify. You will be asked to click on a link contained in that message to enable your account. You will then be able to log in to our Member area and create your special web page!

Prepare your Tribute Page.

Write your testimonial to honor your loved one. Each member may include one photo. Image guidelines will be provided.

When you are finished, make sure to notify friends, family and co-workers that your web page is ready to take donations!

Receive your notification emails.

All members who choose to do so, will receive a noticification email when a donation is made to their fund. Unless the donor has elected to remain anonymous, their name and email address is given. Each member then may acknowledge the donation personally, at their discretion. In any case, all donors receive an acknowledgement letter from ALS of Michigan to document their tax-deductible contribution.

Help empower our pALS and their families through your generous donation. Just choose your fund and use our secure form to enter your credit card information. Your contribution will help provide important resources to accelerate cures for ALS and fund needed research, patient services, public and professional education and community services in Michigan. All your contributions stay right here in Michigan!

You may elect to keep your donation anonymous. While the Tribute Member will not be given your name, you will still receive an acknowledgement letter from ALS of Michigan to document your tax-deductible contribution.

You may also donate to your favorite fund by sending a check made out to "ALS of Michigan" to:
